Getting help quickly – end of life care

Home > Getting help quickly – end of life care

If you are a relative or carer of a person whose condition has got much worse very quickly, they may be eligible for a fast track assessment and care package, if an appropriate clinician determines it suitable to do so.

Having a fast track application means that the funding process for your care happens more urgently.
